Mission
To provide a great healthcare experience to patients and families by:
•Caring for everyone who needs and wants our services
•Establishing trust
•Influencing healthcare providers by sharing our best practices
•Operating with a margin which allows us to be competitive and innovative
•Being a great place to work that fosters a passion for learning, service excellence and embracing change

Vision
To create a world where everyone receives great service at the end of life and no one is afraid of dying.

History
In 1978, Hospice of Palm County’s journey began with just a few patients. As the years went on and the need continued to grow, the 501(c)(3) not-for-profit organization expanded to care for more than 70% of Palm Beach County’s patients and families in need of hospice services.

Today, they are the only provider serving Palm Beach County exclusively, caring for an average 1,200 patients per day. Hospice of Palm Beach County admits and cares for all patients and families who need their services, regardless of ability to pay.

Background
A leading provider of hospice care, Hospice of Palm Beach County serves as a model for other programs nationwide. Services are provided for patients diagnosed with any advanced illness with a limited life expectancy of six months or less. Care is provided in the patient’s residence – whether at home, in an extended care facility, or in a hospital or inpatient unit setting. Hospice of Palm Beach County’s open access philosophy offers patients the benefit of complex, medically sophisticated palliative interventions that are not typically provided by other hospices.

With a team approach to care, each patient has a dedicated Hospice physician – many who are board-certified in hospice and palliative medicine - who works with the patient’s primary care physician, as well as registered nurses, certified nursing assistants, pharmacists, social workers, chaplains, music, massage and other integrative therapists, bereavement counselors and volunteers. The team works tirelessly to ensure each patient has an individualized care plan and that they are comfortable, physically, emotionally and spiritually.

Bereavement services, offered through the John J. Brogan Bereavement Center, offers adult and children’s individual and group counseling programs to anyone in the community who needs and wants these services. We’re there.
